Appointment Policies

  • Appointments can be made via the website(Book via Headway or Patient Portal), email, or phone.
  • The appointment process starts when a prospect or an authorized representative submits the required details on our portal, including names, the service requested, and Cash/ insurance information (if applicable). Our team will review the information and convert the prospect to a patient.
  • The patient will receive an email to activate their account and log in to complete our onboarding forms.
  • Please complete the forms and attach copies of your insurance card’s front and back. This enables us to check your eligibility for the appointment. You must also have your credit/debit/HSA card information available, as you will need them before you can book your appointment.
  • The forms and your insurance card must be received at least twenty-four (24) hours before your appointment, or the system will automatically cancel your appointment.
  • Cancellation/No-Show: Please make cancellations as soon as possible and at least 48 hours in advance, or a fee will be charged for late cancellation/no-show.
  • We accept the insurance listed under Insurance & Pricing. We may be out of your network if your insurance is not listed.
